Reason for Revision Dentistry

Slightly more than one quarter of yearly spending on cosmetic dentistry goes toward redoing unsatisfactory work from a previous dental visit (estimated by the American Academy of Esthetic Dentistry). Many of these problems associated with bad cosmetic dentistry can be unsightly as well as painful and unsafe.

Revision dentistry focuses on failed crowns, dental implants, veneers, fillings or another type of poorly accomplished cosmetic dentistry. Other issues may result from incorrect bonding of veneers, or placing crowns or dental implants that do not match the patient’s mouth or biting surface, resulting in discomfort and health concerns.

Common reasons for failed dentistry include:

Revision Dentistry Consultation

After completing your initial comprehensive dental evaluation for revision dental work, Dr. Mabrito will communicate the process and expected results of the procedure(s) to ensure your expectations are understood.  You will view before and after photos of patients with similar conditions and Dr. Mabrito can simulate the final result with computer imaging.

It is helpful if you bring in photographs of what your smile used to look like or photos from magazines that depict your ideal smile. Computer imaging is used to create an image of your new smile to eliminate any concerns about the anticipated end result. Dr. Mabrito will also create a wax model to show how your teeth will look after the revision.

Patients should keep in mind that every jaw and facial structure is different, so what you have in mind may be different from your achievable results. You can discuss any of these concerns with Dr. Mabrito.

Process for Successful Results

For procedures that will have an effect on your bite and jaw function, such as extensive crowns, implants or veneers, Dr. Mabrito applies a temporary resin to the tooth to simulate the final results. You wear this material for a week or two until your jaw is comfortable with the change.  If your jaw does not become accustomed to the change, adjustments will be made until a comfortable setting is achieved.  Proper placement is important to avoid further problems such as TMJ.

Poorly Placed Porcelain Veneers
Proper placement of porcelain veneers can offer patients a beautiful, long-lasting smile. If the veneer was improperly designed, constructed, or bonded to the teeth, problems such as cosmetic and structural failure can occur.

Bonding Completed Improperly - Veneers can fail when improperly bonded to teeth or exposed to contamination during placement.

Failing or Improperly Placed Dental Implants

Placing implants is a highly technical, multi-step process and when done correctly is an excellent option for replacing missing teeth. Issues with Aesthetics
A single front tooth is the most difficult implant to place and mistakes tend to be more frequent. Aesthetic issues associated with front implants may include improper alignment with the arch of surrounding teeth and visibility of the metal post.

Issues with Bone
Periodontal disease or a situation where a bone graft should have been performed can cause gum recession and implants to become loose due to the jaw’s inability to stabilize the implant.

Loose or Broken Implants
Properly placed dental implants should stay stable for life. Any loosening is a sign of failure and must be treated immediately. Factors contributing to implant loosening include trauma, injury, bone issues, gum disease, or other oral health concerns.  These problems are often impossible to predict or may be the result of poor judgment lack of training by the previous dentist.

Implant Failures – Early Stage
Errors during the initial implant placement include: overheating the bone while placing the titanium post, applying too much force, or contamination of the implant's titanium anchor.

Implant Failures – Late Stage
Late failures also occur during the implant procedure but do not become apparent until later. Teeth clenching, bite shifting, or changes in the vertical dimension of the jaw can all lead to fractures from the excessive force put on the implant.

Crown Failures
Crowns that have been improperly constructed or bonded can become loose and uncomfortable. If your crown was improperly made or bonded and extends too high bite pain can occur. Failing crowns can be extremely painful, pose an increased risk for infection and can cause major injury to the affected tooth. Dr. Mabrito also replaces older crowns with metal and unsightly margins with all-porcelain crowns.

Aged Restorations
Revision Dentistry can also be used to restore older dental restorations, for example a crown placement 30 years prior. Old fillings may need to be replaced because they have cracked or popped out or you may want to remove the visible silver in your mouth. Over time metal eventually blackens from oxidization and will make your fillings appear darker. Dr. Mabrito can replace these old fillings with tooth colored fillings.
